Appointment and Prescription Refill Information

 

About Your Visit

On your first visit, please bring a list of all the medications and supplements you are currently taking. Advise us if you have any allergic reactions to any medications. Each time you meet with your provider make sure to ask them if you need refills.

If you are a new patient, try to obtain your previous medical records, including recent laboratory tests, prior to your appointment.

Please arrive 15 minutes early for your appointment so we can check you in.

If you are unable to keep your appointment, please call us at least 24 hours in advance, as a courtesy to other patients who may need urgent care. Please make every effort to keep your appointment, as "no shows" inconvenience both other patients and your provider.

Remember to bring all prescription and over-the-counter medications that you continuously take or you are currently taking. Include all medications given to you by other physicians.
